Connecting Choices with Care

We empower individuals to live their lives with dignity and autonomy.

We are committed to supporting people's choices, ensuring they have the freedom to decide how they want to live and how they want to be cared for during what can be a vulnerable part of life. Our team of dedicated professionals provides a range of services designed to meet the unique needs of each individual, helping them navigate the complexities of the health and aged care system. We strive to prevent individuals from falling through the cracks, ensuring they receive the care and support they deserve.

Our goal is to create a compassionate and inclusive environment where everyone can age with grace and independence.

You can also keep up to date with any changes by visiting the Support at Home Government Page, where you can register to receive aged care newsletters and alerts. Alternatively, there are fact sheets and resources via My Aged Care.

Additionally, the Australian Government introduced the Aged Care Bill 2024 to Parliament on 12 September 2024.

The new Aged Care Act will start from 1 November 2025. It will be the main law that sets out how the aged care system operates.  This plain language fact sheet available in 22 languages, gives a summary of each chapter of the Act. It also explains what the government is doing to support the changes now and in the future.
